What is a Subject Access Request (SAR)?

 A Subject Access Request (SAR) is a request you can make to the practice to access the personal information we hold about you. This includes medical records, appointment history, and other personal data we keep in relation to your care.

How long does it take?

By law, we aim to respond to SARs within one month of receiving your request.

Why might it take a long time?
 

Processing SARs can take time because:

  • We need to carefully check and compile all your personal information.
  • We must ensure sensitive information, such as details about other individuals, is protected.
  • Large requests or older records may require extra work to locate and review.

Why can’t we include third-party information?

If your records contain information about other people, we cannot automatically share it with you without their consent, as it is their personal data. This is to protect the privacy of others.

Proof of identity is required

To process your request safely and securely, we can only provide information if you supply valid identification, such as:

  • Driving license
  • Provisional driving license
  • Passport

Without valid ID, we cannot release your personal information. This is to ensure your data is only shared with the right person.

Other important points:

  • We may ask you to clarify exactly what information you need if your request is very broad.
  • You will receive the information securely, either electronically or in printed form, depending on your preference.
