Self-refer to the midwives

Please self-refer directly to the midwives. The GP will be unable to refer you to a midwife. We recommend that you make an appointment with the midwife as soon as possible. Ideally, your booking appointment should be carried out before you are 10 weeks pregnant for the best outcomes for you and your child. Please click on the link below to self-refer.

If you have any of the following conditions then we advise you contact the practice to book a GP appointment. Please do still complete the above self-referral form to the midwives, as we will not be able to refer you.

  • Epilepsy
  • Diabetes
  • High blood pressure
  • Sickle cell anaemia/ Thalassemia
  • Under/overactive thyroids
  • Heart disease
  • Blood clots in leg/ lung OR family history of clots
  • Mental health conditions
  • Uncontrolled asthma

If you are under 18 years old, please book a GP appointment. Please do still complete the above self-referral form to the midwives, as we will not be able to refer you.

We would advise you to kindly start taking Folic Acid (400mcg once daily) and Vitamin D (10mcg daily). These can be purchased over the counter from pharmacies or through Healthy Start schemes. For some health conditions – such as those above – a higher dose of Folic Acid (5mg) may be required which your GP can prescribe.
